On behalf of our Chief, I am formally requesting you to complete the 2019 Chemical Survey so we are able to meet our obligations to our firefighters under MIOSHA. It is mandatory that you complete the survey. Even if your facility does not use, produce, or store any of the listed chemicals you are still required to complete the survey.
The Michigan Fire Prevention Code, PA 207 of 1941, as amended, requires any facility handling hazardous chemicals to provide information relating to the chemicals to the jurisdiction’s fire chief upon their request. The purpose of the survey is to determine the quantity of specific chemical groups used, produced or stored in your facility. The information collected in the survey is beneficial to you and the firefighting personnel who respond to a fire or other emergency at your facility.
Instructions: On the survey, indicate whether your site uses, produces, or stores any of the chemical types listed. Check all the categories that apply. Each chemical group listed in the survey includes a specified quantity. Indicate the quantity category for each chemical group on your site. To complete this survey, you may need to reference Material Safety Data Sheets, SARA Title III reporting forms, along with the attached definitions
